PATH Report - Persons Served During Report Period
The following sections are meant to help connect how different rows of the PATH Report in the Persons Served During This Reporting Period section are connected to the data entry in HMIS, specifically contacts, date of engagement, and PATH status determination.
Row #8: Number of Persons Contacted by PATH-Funded Staff this Reporting Period
This line in the report shows the number of clients who have at least one contact recorded in their HMIS record during the reporting period. A client will show in this row, even if they have a Project Start Date for a time before the specified Program Date Range* of the report. A client will also show in this row, even if they do not yet have a Date of Engagement or Enrollment Date.
Specific Data Elements to Populate Row
- Project Start Date (does not need to be within the Program Date Range* of the report)
- Contact (i.e. Current Living Situation) at least one recorded in the clients HMIS record
Rows #9 and #10: Number of New Persons Contacted This Reporting Period in a PATH Street Outreach (Line 9) or a PATH Services Only Project (line 10)
These two rows in the report show the number of new clients who have at least one contact recorded in their HMIS record during the reporting period, and it separates out the clients entered into the Street Outreach component (Line 9) versus the Services Only component (Line 10). A client will show on these rows only if they have a Project Start Date that is also within the specified Program Date Range* of the report (this is different than Line #8). A client will show in these rows, even if they do not yet have a Date of Engagement or Enrollment Date.
Specific Data Elements to Populate Rows
- Project Start Date (must be within the Program Date Range* of the report)
- Contact (i.e. Current Living Situation) at least one recorded in the clients HMIS record
**** Please remember that the distinction between Street Outreach and Services Only is where the client sleeps most often when you initially contact them. If the client is sleeping in a place not meant for habitation more often than not, the client should be entered into the Street Outreach component. If the client is sleeping in shelter or some other sheltered location more often than not, the client should be entered into the Services Only component. A single client should never have an entry into Street Outreach and Services Only at the same time. ****
Row #11: Total number of New Persons Contacted This Reporting Period
This row in the report shows the total number of new clients who have at least one contact recorded in their HMIS record during the reporting period. It is adding up the client counts in Lines #9 and #10. Clients must have a Project Start Date within the specified Program Date Range* of the report to appear in this line (same as Lines #9 and #10).

- Contact (i.e. Current Living Situation) at least one recorded in the clients HMIS record
Row #12: Instances of Contact, from Frist Contact until the Date of Enrollment, for All Active Clients Who Became Enrolled This Reporting Period
This row in the report shows the number of contacts for clients who have a Date of Engagement and Date of Enrollment that fall within the specified Program Date Range* of the report. This means that the client must have a Date of Engagement and Date of Enrollment recorded in their HMIS record. Additionally, if a client has more than one contact recorded between their Project Start Date and Engagement and Enrollment Dates, the client will show on this line multiple times for the multiple contacts recorded.

- Date of PATH Status Determination (Enrollment) (must be within the Program Date Range* of the report)
Row #13: Number of New Persons Contacted This Reporting Period Who Could Not Be Enrolled Because of Ineligibility for PATH
This row of the report shows the number of new clients who were contacted during the specified Program Date Range* who were determined ineligible for PATH and could not be enrolled. This means that a client must have a Date of PATH Status Determination within the Program Date Range* but that PATH Status Determination is a No answer. Additionally, the If no, reason not enrolled answer must be Client was found ineligible for PATH.
Specific Data Elements to Populate Line
- Project Start Date (must be within Program Date Range* of the report)
- Contact (i.e. Current Living Situation) at least one recorded in the clients record
- Date of PATH Status Determination (must be within the Program Date Range* of the report)
- Client Became Enrolled in PATH (must be a No answer)
- If No, Reason Not Enrolled (response must be Client was found ineligible for PATH)
Row #14: Number of New Persons Contacted This Reporting Period Who Became Enrolled in PATH
This row of the report shows the number of new clients who were contacted during the specified Program Date Range* who also became enrolled in PATH during the reporting period. This means that a client must have a Project Start Date, Date of Engagement, and Date of PATH Status Determination that fall within the reporting period.
Specific Data Elements to Populate Line
- Project Start Date (must be within the Program Date Range* of the report)
- Contact (i.e. Current Living Situation) at least one recorded in the clients record
- Date of Engagement (must be within the Program Date Range* of the report)
- Date of PATH Status Determination (must be within the Program Date Range* of the report)
- Client Became Enrolled in PATH (must be a Yes answer)
Row #15: Number with Active, Enrolled PATH Status at Any Point During the Date Range
This row of the report shows the number of clients with a Date of Engagement and Date of PATH Status Determination completed, and the Client Became Enrolled in PATH as a Yes answer, regardless of when those data elements were completed for the client, as long as the client was active during the specified Program Date Range*. If the client does have a Project Exit Date, the time between the clients Project Start Date and Project Exit Date must fall within the specified Program Date Range*.
