PATH Report - Referrals Provided

The following section of this guide is meant to help connect how different lines of the PATH Report in the Referrals Provided section are connected to the data entry in HMIS, specifically contacts, date of engagement, and referrals.

**** Please remember that the only clients for which the Referrals Provided section of the PATH Report will populate are those who have a Date of Engagement and Date of PATH Status Determination completed. ****

 

Type of Referral

The Service Transactions dashboard on the clients HMIS record is used to record Referrals. Remember that Referrals are those supports that the PATH-funded agency does not provide in-house to clients and so they rely on community partners to provide those supports to clients through a referral.

First select at least one Need the reason for which you are making this referral to the community partner.

Select the DHS North Dakota Department of Human Services PATH Referrals in the Referral Provider Quicklist menu and Add Provider.

Select a Type of PATH Referral. If you do not select an option from this list, the referral WILL NOT populate in the PATH report.

Specific Data Elements to Populate Lines

  •       Project Start Date (does not need to be within Program Date Range* of the report)
  •          Contact (i.e. Current Living Situation) at least one recorded in the clients record
  •          Date of Engagement (does not need to be within the Program Date Range* of the report)
  •          Date of PATH Status Determination (does not need to be within the Program Date Range* of the report)
  •          Client Became Enrolled in PATH (must be a Yes answer)
  •          Referral on Service Transactions Dashboard (must be recorded within the specified Program Date Range* of the report)
  •          Type of PATH Referral (MUST BE POPULATED with one of the options in the list)
