PATH Report - Services Provided

The following section of this guide is meant to help connect how different rows of the PATH Report in the Services Provided section are connected to the data entry in HMIS, specifically contacts, date of engagement, and services.

**** Please remember that the only clients for which the Services Provided section of the PATH Report will populate are those who have a Date of Engagement and Date of PATH Status Determination completed. ****

Type of Service

The Service Transactions dashboard on the clients HMIS record is used to record Services. Remember that Services are those supports that the PATH-funded agency provides in-house to clients. There are two places you must record the service a client receives in order to populate the lines on the PATH Annual Report.

The first is the Service Type* dropdown list.

The second is the Type of PATH-FUNDED Service Provided. If you select the Service Type* but you do not select the Type of PATH-FUNDED Service Provided, the service WILL NOT populate in the PATH report.

Specific Data Elements to Populate Lines

  •          Project Start Date (does not need to be within Program Date Range* of the report)
  •          Contact (i.e. Current Living Situation) at least one recorded in the clients record
  •          Date of Engagement (does not need to be within the Program Date Range* of the report)
  •          Date of PATH Status Determination (does not need to be within the Program Date Range* of the report)
  •          Client Became Enrolled in PATH (must be a Yes answer)
  •          Service Type* on Service Transactions Dashboard (must be recorded within the specified Program Date Range* of the report)
  •          Type of PATH-FUNDED Service Provided (MUST BE POPULATED with one of the options in the list)
