ShelterPoint - Entering an Individual into a Project

1.      Log into ServicePoint.

2.      Click Enter Data As select the project you for which you are entering.

3.      Click on ShelterPoint.

4.   Once in ShelterPoint, confirm the provider to be the project that you are entering the client into.

5.      From the Unit List, select the unit in which you want to enter the client into and click Submit.

6.      From the ShelterPoint Dashboard click on Client Check In,

7.      This will bring you to the Shelter Inventory Information. Here is where you can select the Room and bed that the client can be entered into.


8.      Click on the green check-in symbol next to the bed you want to enter the client into.

9.    You will then need to search for your client by using the Client Search. Enter in the clients first and last name and set the Name Data Quality to type of name being searched. Click Search. If the client is found click on the green plus sign next to the clients name, if the client is not found click on Add New Client with this information.

  

10.  You will then be brought to the United Entry Data form for your client. Fill in the following areas accordingly:

a.      Required: Date In: manually set the date as to when the client checked in. Please set the time to 12:00:00 am.

b.      Optional:

                                                              i.      Supplies Given

                                                             ii.      Locker Number

                                                           iii.      Codes/Notes

c.       Release of Information

                                                              i.      Set the release of information accordingly by clicking View ROI.

                                                             ii.      Click on Add ROI

                                                           iii.      Set the following points

1.      Provider

2.      Release Granted

3.      Start Date and End Date

4.      Documentation

5.      Witness

                                                           iv.      Click on the binder clip to attach the signed document (see ROI How-To Guide)

                                                             v.      Save Release of Information

                                                           vi.      Exit

11.  Entry Data

a.      Complete all components in the assessment. The assessment that is listed has been set based on the project type


12.  Once you have completed the required assessment; click Save and Exit and you will be returned to the Shelter Inventory screen where you will be able to see the client entry into the bed you have selected.

You have now completed a successful entry into the North Dakota HMIS System.
