When To Complete And Enter An Annual Assessment in NDHMIS
An Annual Assessment must be created for clients who are enrolled in certain project types for a year and longer. Those project types that require annual assessments are ones that are required to collect the following data elements: Income and Sources, Non-Cash Benefits, and Health Insurance.
To be considered completed, the Annual Assessment must be recorded in AKHMIS within the period of 30 days before or 30 days after the annual anniversary of the client's entry into the project. For guidance on how to complete an Annual Assessment, please contact NDHMIS Help Desk.
