NDHMIS Release of Information Documentation Flowchart

The below flow chart will assist in determining if a client needs to have a new shared client record created in HMIS, an unshared client record created in HMIS, or if an existing shared client record can be used. Some points to remember:

All clients that appear within the search results have consented to share their information within NDHMIS and the signed ROI should be attached and visible under the File Attachments within the Client Profile tab. Both pages of the signed ROI are required to be attached.

Beginning June 1, 2023, when a client signs the Client Informed Consent and Release of Information, and they select either Option 2 (Limit sharing) or Option 3 (do not share) you will need to submit the Record Visibility Adjustment Request form found on the ICA North Dakota website under the Additional HMIS Resources.  Moving forward, the North Dakota Help Desk team will be making visibility changes on all client records in an effort to reduce the duplication of records within the system.  We are committed to report the most accurate data as possible.  

By default, the following types of projects have client records set to unshared when created in NDHMIS. If the client wishes to share their information within the system, a new shared client record will need to be created.

o   Housing Opportunities for Persons With AIDS (HOPWA) Projects

o   Runaway Homeless Youth (RHY) Projects
